Burial Ground Forms

These forms are the most up to date and must be used in conjunction with the burial process flowchart. Out of date forms will not be accepted.

  1. Form BG1 – Notice of Interment Coffin Burial
  2. Form BG2 – Notice of Interment Cremated Remains
  3. Form BG3 – Purchase of Exclusive Right of Burial (ERoB)
  4. Form BG4 – Assignment of ERoB to a new owner
  5. Form BG5 – Transfer of ERoB Original Owner Deceased
  6. Form BG6 – Application to install/update a memorial
  7. Form BG8 – Application for a memorial permit (Masons and Funeral Directors only)
